Athletic Participation Forms
We are very excited to announce that the Mason Athletic Department has partnered with an online program called FinalForms, which will let you and your student athlete complete & file most athletic forms electronically. FinalForms is available for all Comet athletes at https://mason.finalforms.com. The only form that you are still required to actually print and file is the two page OHSAA Physical form that must be completed and signed by the doctor (by law we are required to have a signed copy on file). Once your student’s physical is received by the athletic department, it will be put into FinalForms and you will receive automatic notifications when your need to update your student’s forms.
1) Read the step by step Parent Registration Guide
2) Log onto Final Forms and Complete Paperwork with digital signatures. Make sure your athlete signs the forms as well.
3) Schedule a physical with your doctor
4) Return signed Physical Form with doctor’s signature to the Athletic Department or appropriate coach.
Pay to Participate
Athletes are required to pay a fee for each sport they play, look here to see when and how much is to be paid for your students sport
Alternate Transportation Forms
Mason City School District will be providing transportation to or from this activity. However, due to circumstances, my child will not be able to utilize this option.
Mason City School District will not be providing transportation to or from this activity. Thus, if i choose to allow my child to participate in this activity, I acknowledge that it is my responsibility to ensure that my child is provided safe transportation to and from this activity.
Information regarding Volunteer Drivers
Transcript Release Form
This form is to verify that William Mason High School has permission to release a transcript of grades and/or college board (ACT or SAT) test scores regarding the student athlete to any coach/recruiter that requests this information, until the student graduates.
The Mason High School Athletic Department recommends that all students athletes have their annual Pre-Participation Physical Examination conducted by the student athlete's primary care physician. In the event that the student athlete and/or his/her parent(s) or guardian(s) elect not to utilize their primary care physician, pre-participation physical exams can be obtained at the Premier Family Care of Mason located on the campus of Mason High School.
